approach her about it because no one deserves that kind of treatment, let her know it is unacceptable and if you feel you could make it work with her after this, let her know. BUT make it on condition that she is faithful to you. She is putting your health at risk and not respecting you. Maybe do something to show her that you love her... even tho it hurts

I don't think that the last line makes it ring untrue. You sound like you are probably in your 40s and this is not your first serious relationship. Your passivity probably feels safe for you - and is likely also comfortable for your wife - however lack of passion might be the motivation for her straying.
